New patient portal

At our practice keeping our patients and staff safe is one of our top priorities. To reduce face-to-face contact at the practice, we are very excited to be able to offer our patient portal.

Our new patient portal allows our patients to complete all their relevant forms at their own leisure before attending the practice for their appointment.

To complete your forms it is very easy and simply. You will receive a SMS/email appointment reminder which will also include a link to our patient portal.

Click on the link and follows the easy steps to complete your forms.

Once all yours forms are completed, when you arrive at the practice for your appointment all you need to do is hand sanitise and reception will let you know which waiting area to take a seat in.

7  Easy steps

• Click “fill out forms” button/link in your reminder appointment email/sms.
• Log into your patient portal with just your surname and date of birth.
• Once signed in, you will see your forms which need to be completed.
• Click on your first form to begin.
• Once you have completed your forms click save.
• Proceed and follow the same steps for the rest of your forms.
• All completed forms will show on your patient portal homepage.
• Your now ready to attend your appointment.
